WesleyLife Home Health provides a full range of home health care services across central Iowa, serving clients in Boone, Dallas, Hamilton, Jasper, Marion, Marshall, Polk, Story, Warren, Cass, Washington, Audubon, Guthrie, Adair, and Poweshiek counties. Our services include medical care, therapy, and assistance with daily living, all delivered in the comfort of clients' homes to improve quality of life, foster independence, and promote overall well-being. A Day in the Life as a LPN Visit Nurse with our Home Health team:

• Provide nursing care based on care plans, traveling within the service area in all weather conditions.
• Monitor clients’ health, report changes, and document care updates in the electronic medical record (EMR).
• Participate in on-call rotations, including evenings, weekends, and holidays.
• Join nursing case conferences, team meetings, and complete required training.
  
  

Open Shifts Available: Full-Time Monday-Friday with rotating on-call (weekends)
